A brief description of the usage of each form and guide is also included to assist you in selecting the right document for your needs. Employer Enrolment FormThis is for employers wishing to join the Accountants Super. There is no charge for registering with Accountants Super. |

Employer Direct Debit Request FormFor employers to establish Direct Debit arrangements with Accountants Super |

Tax File Number ReportFor employers to provide employees' TFNs at their request. |

Super Choice FormFor employers to give to new employees to ensure SG contributions are paid correctly for all employees. |

Change of Employer Details FormUsed by employers to notify a change in their contact details and company name. |

Contribution ReturnThis blank contribution return can be used to remit contributions for employees. |

New Member Enrolment and Initial Contribution Remittance FormEmployers enrolled in Accountants Super can use this form to "bulk" enrol new employees and to provide details of initial contributions remitted for new and existing employees. |

Notification of Exited Members FormEmployers should complete this form to notify Accountants Super of members who have terminated employment. We will then contact the employees listed on the Form. |

Stationery Order FormEmployers who wish to order supplies of stationery should complete this form. The stationery will then be mailed to the nominated person. |
