Member forms and guides
The Trustee has designed a number of easy-to-use forms to enable members to easily transfer other benefits into Accountants Super, to advise us of changes in their insurance requirements, investment options, personal details and when leaving Accountants Super, of their benefit payment requirements. The forms for these purposes and others shown below can be downloaded from this website.

Member forms
Application for Membership FormThis form is for members wanting to join Accountants Super, including those who have been enrolled by their employer and those joining as a member's spouse. |

Super Choice FormThis form is used by eligible members to instruct their employers to send their contributions to Accountants Super. The Accountants Super Letter of Compliance is also included to identify Accountants Super as a complying fund that can accept employer contributions. |

Voluntary Contributions FormThis form is for members wanting to make a voluntary contribution, especially to be eligible for a Government Co-contribution. |
|
Change of Member Details FormThis form can be used by members to notify a change of address, employment status, name, employer or other changes to personal details. |
|
Nomination of Beneficiary FormUsed by members for nomination of dependants in the case of death. |
|
Direct Debit FormUsed by members who wish to make Member Voluntary Contributions through Member Online Sevices. |
|
Death and TPD Insurance Opt Out FormUsed by members who no longer require death and total and permanent disablement insurance. |
|
Insurance Application FormMembers who wish to increase their death and total and permanent disablement benefits should complete this insurance application form. |
|
Investment Choice formThis form allows members to change their investment options at any time. You can change investment options for your current balance, future contributions or both. |

Transfer FormMembers who wish to transfer benefits held in other funds to Accountants Super should complete this form. Alternatively, you may use OnlineExpress to populate this form. |
|
Income Protection Application FormUsed by member wanting to insure their income against illness or injury. |
|
Tax File Number (TFN) Notification FormMembers who did not notify Accountants Super of their tax file number when first joining who subsequently wish to do so should complete this form. |

Consent to Search FormMembers who would like us to assist them in searching for any old super accounts can complete this form. If you have had a number of jobs and moved house a few times, you may be surprised at how much missing super you have. |

Third Party Authorisation FormMembers who would like a third party (e.g. their adviser or accountant) to have access to their account need to send this form to us. This authority can be revoked in writing at any time. |
|
Third Party Authorisation Withdrawal FormMembers who no longer want a third party (e.g. their adviser or accountant) to have access to their account can cancel their authority by completing this form and returning it. |
|
Benefit Withdrawal Form The Benefit Withdrawal Form is used by members who are able to claim their super. To obtain a copy of this form and proof of identification requirements for your situation, please call our Customer Service Centre on 1300 651 331. |
