Super choice form - take Accountants Super with you
Instruct your employer to send their contributions to Accountants Super.
The Accountants Super Letter of Compliance is included. This is required to identify Accountants Super as a complying fund that can accept your employer's contributions.

Tax File Number (TFN) Notification Form
Tell us your tax file number.

Personal statement - Death & TPD
If you are applying for death and total and permanent disablement insurance totalling more than $1,000,000, or have answered "Yes" to any of the questions on the Insurance Application Form, you will need to complete this Personal Statement.

Income protection application form
Apply to insure your income against illness or injury.

Personal statement - Income protection
If you are applying for Income Protection cover totalling more than $8,000 per month, or have answered 'Yes' to any of the questions on the Income Protection Application Form, you will need to complete this Personal Statement.

Death and TPD insurance opt out form
Cancel your death and total and permanent disablement insurance.

*If medical testing or health information is required in relation to a request for insurance, Accountants Super may arrange for you to be contacted by Lifescreen. Lifescreen is a mobile health service provider who can come to your home or office at a time convenient to you to perform medical tests required by the fund's insurer, CommInsure, in order to assess your application for insurance.

Accessing your super

Our Benefit withdrawal form is used to access your super. To obtain a copy of this form and proof of identification requirements for your situation, please contact our Customer Service Centre on 1300 651 331.
